How It Works
High earners enrolled in Medicare Part D pay more than the sticker premium, and this tool estimates that extra charge, the Income-Related Monthly Adjustment Amount (IRMAA), for 2026 based on your Modified Adjusted Gross Income (MAGI). Since the surcharge stacks on top of whatever your standard Part D plan already costs, seeing the number ahead of time lets you fold it into your budget instead of meeting it as an unwelcome surprise.
The Social Security Administration sets your 2026 Part D IRMAA from your MAGI two years back, typically your 2024 tax return, and applies it in tiers. Once your income clears a threshold, the surcharge goes directly to Medicare and climbs step by step as income rises. For 2026, the first tier kicks in when MAGI passes $109,000 for an individual or $218,000 for a couple married filing jointly, with each higher bracket adding more.
The output is an estimate; the SSA fixes your actual IRMAA from your official filings, so treat these numbers as a planning guide. Life events such as marriage, divorce, or a sharp change in earnings can shift your MAGI and the surcharge that follows it, which is worth revisiting when circumstances change. Lowballing your income on purpose tends to backfire, since the real figure can push you into a higher tier than expected.
