How It Works

IRMAA, the income-related monthly adjustment amount, is a surcharge on Medicare Part B and Part D premiums. In 2026 it applies once modified adjusted gross income passes $109,000 for a single filer or $218,000 for a married couple filing jointly. Social Security uses the tax return from two years earlier, so 2024 income sets 2026 premiums. MAGI here is adjusted gross income plus tax-exempt interest.

The brackets are cliffs, not marginal rates: one dollar over a threshold charges the full next surcharge for the whole year, and a married couple who are both on Medicare pay it twice. In 2026 the standard Part B premium is $202.90. The five surcharge tiers add $81.20 to $487.00 a month to Part B and $14.50 to $91.00 to Part D, per person. The calculator shows how much more income fits below your next threshold, which is the number to check before a Roth conversion, a capital gain or a large IRA withdrawal.

Married people who file separately and lived together at any time during the year have only two thresholds, $109,000 and $391,000. If income fell because of retirement, the death of a spouse, divorce, marriage or a lost pension, Form SSA-44 asks Social Security to use a more recent year. The 2027 thresholds are indexed to inflation and come out with the 2027 premiums in November 2026; the $500,000 and $750,000 top thresholds stay frozen until 2028.

Example: $150,000 of MAGI and a $30,000 Roth Conversion

  2. 2 2026 bracket: $150,000 falls in the $137,001 to $171,000 tier. Part B costs $405.80 a month ($202.90 plus a $202.90 surcharge) and Part D adds $37.50, so the surcharges come to $2,884.80 a year.
  3. 3 Headroom: $21,000 more income fits before the $171,000 threshold.
  4. 4 Result: the $30,000 conversion lifts MAGI to $180,000, the next tier ($324.60 Part B and $60.40 Part D surcharges), so premiums two years later rise by $1,735.20 for that year. Converting $21,000 instead would stay in the current tier.
